2. Physical Examination Form & Sickle Cell Trait Verification Form
3. Sickle Cell Trait NCAA FAQs
4. Insurance Information
5. ADHD Medical Documentation (as necessary)
6. 2023 Health Screening Schedule (To be completed upon arrival on campus)

When at all possible, please submit completed forms via SportsWare. If mailing the completed physical exam form, sickle cell verification form, copies of insurance card, ADHD medical documentation, or additional surgical/rehabiliation notes is preferred, they can be mailed to:
